If phone doesn't goes into "Qualcomm...9008" Mode (QM) automatically, You can try to put it in QM by the next way: disconnect battery, press and hol "Vol+ "+ "Vol-" and connect USB cable.
If device can't enter in QM with key combination, try to use EDL cable.
If still not done - disassemble the phone and and try to find and use Test Point.
