You can identify it in the same way as older versions *#0000#
To revive this phone simply flash it with sl3 fw version and will be ok (but first you need to be sure it was sl3). Do you have identify log readed with Jaf main before unlocking attempt ? Or unlocking log from Jaf bb5plus no tp 1.02 ? You can see in these logs which version it was...
