for read codes(unfreeze code too) use only USB cable and option READ UNLOCK CODES selected.
For direct unlock U need COM&USB cable (combo) and select only UNLOCK PHONE option.
What cable are U using? This error happen when MSL can't be initialized, or U use USB cable and trying direct unlock.
