unlocked fly slt100 not work on O2

i have followed the instructions to unlock the fly slt100 and having reflashed it it only works on uk networks orange, vodafone, t-mobile and virgin but not O2. any idea's how to get this to work?

I got an SLT100 flashed with Hummer software and works with some Orange sims, but not with others shows a picture of a funny shaped SIM Card.
IMEI is correct
