need steps to unlock lg cu515

How do I unlock this lg cu515 phone from att. I received the 16 digit unlock code from att but not the steps to physically unlock it. Any step by step advice?

Model : CU515

1.Put a SP-locked sim in the phone, phone say invalid smart chip
2.Now enter *#865625# then select option 1 and then again select 1 and enter your 16 digits code
Phone will be unlocked!
