S621x Alcatel Always Fomatting ERROR

Am trying to format this Alcatel as it stuck on Alcatel logo...
i previously did to many phones with such state and all went ok after formatting no need flashin..but it always stops at this stage log BELOW

wat cen be reason whereas phone flashes ok without any prob..
i did try with reduced speed but no vain

1. latest MModule version is 2.57 but not 2.49 !
2. if you see the same error during format several times, it may be phone hardware fault, f.e. damaged or damaged flash chip

and also with the flash file recommended by UMER and post results soon thankx
