u erase ur phone fully so u flash ur phone all complete u one think missing. when u erase ur phone erase ur pm (permanent memory) file also. i check ur flash log so i confirm u lost ur pm file. now u flash ur phone than write pm ur phone will be ok

only unlock it with jaf box

by log it seems ur phone is not locked.. do following things

first just do unlock. if not help then

1. erase full with JAF
2. falsh ur phone with RM-225 firmware
3. write good PM( search or contact me if u need it)
4. Unlock
