Need major help with nokia 6101

Hi, I've posted several posts about my phone being restricted but i still haven't found help.

When I put in the sim card and enter my pin it asks for a restriction code.
I've tried several codes but none work.

Imei is 357589003736981
Type RM-76
Made in hungary and inside the phone there's written "T-MOBILE D NET"

I dont really know what country the phone is locked to, but there's T-Mobile written on the phone.

Second question is when I enter the codes with sim card inside do I just enter the code or do I add the pw+ sings too ?

When I enter the code with the pw+ signs without the sim card in phone nothing even happens, it doesn't do anything.

Or maybe when I put in the codes with the simcard in phone it doesnt work because i have tried over 5 times ???

Who can help ?
