I had this problem, symptons were:

1) vol- button not working with phone turned on (but works to enter recovery);
2) led flash light turns on when pressing power (with powered off phone), rebooting phone, and when battery was inserted;
3) safe mode did not go away even after repair efs, imei, product code and sn.

reballed camera IC as told by czaren, now problem is gone! thanks a lot, I would never think about that.
