@ kamran1,

According to the log file you have posted the problem appears due to hardware malfunction.
There are 2 possible reasons for such problem to appear:
1. The flash memory chip is damaged or doesn't communicate with the processor. In this case the phone needs hardware repair. Connection between the chip and the PCB may be improved by the hot air.
2. Testpoint procedure was not performed properly - just to make sure please, repeat the TP procedure.
