what do i need to do to unlock this fone i need the procedure,or may somebody explain from the picture below

sir where do i find this FAQ ,i have looking for it in this shell but can't find it,but according to the picture do i have the credits,what the meaning of counter 3& 4 being 15 each

I am trying to unlock my treo 680,and this is what i get.The unlock process may take up to 20 seconds,please be patient.done but seen no unlock data. any help?
