redmi note 3 flashing error qlm module

hello sir

i face this error 2 time while flashing redmi note 3 4g 16gb on x86 pc

1. Power Off Phone , Remove battery , Insert back
2. PRESS and HOLD BOTH VOLUME KEYS!
3. Insert USB cable. In some cases require use EDL cable or TP to force EDL mode!

after this phone started ok but have some option removed like

1. no keypad in message and at other places where it should pop up for writing
2. no wallpaper in phone
3. some ringtone playing and some not
and may be other that are not found.

customer was in hurry so i flashed with miflashtool and phone becomes complete ok.

can you please figure out why this happen?
