First make full backup of your phone.
Did you try with other unlock method, like smart-fast, standard fast?
Also you can try repair security area option.

P.S. this model Metfone-MF6201 still under test

Please, try to flash your phone with different ZTE-R221 flash files from support area.
What is the results ?

Also, do you have original full flash backup file ?

Please, make this test:
1. flash phone with different R221 firmware files from support area and see results.
I think one firmware should be good for your phone
2. if still no luck, try to flash phone with SFR-115 flash files one by one.
