Flash phone with any "repair eeprom" file from support area and then make "Repair" operation.

Is it was good working or dead phone ?

Hello,

As I can see from your previous log phone has problems after you have flashed it (model mismatch after flashing).
This means that your phone has hardware problems. You must open phone and repair it's hardware.
