Download and istall OTU UPDATER
Try to connect usb cable while holding both volume key (on powered off phone),
see if it will install drivers, then start otu updater, select model (ot-996 or ot-995),
Let the updater fix it for you.

You can try with ot-995 custom recovery, download it HERE,
Download custom 4.0.4 rom - HERE, Unpack content to sdcard - should have this path :
\clockworkmod\backup\2012-10-03.15.27.55\

Flash custom recovery - fastboot flash recovery cwm_touch_cocktail_6.0.2.7-testing-V2.img
Reboot to recovery, with sdcard (which have unpacked rar file), go to backup/restore and RESTORE content.

Hopefully it will boot up.
