Samsung T519 from T-Mobile "Phone Locked, Return for Service"

I have a customer who has a Samsung T519 from T-Mobile and it won't accept the unlock code and become permanently blocked.

Here is the problem as described by the customer:

When I turned on the phone, it said "Wrong card" instead of "Password". It took me awhile to realize that this was the equivalent to "Insert Correct SIM card" in your online instructions. Once I realized this, I punched in #0111*12392461# as per the online instructions (12392461 was the unlock code you sent). When I tried the same procedure again, it locked me out.
"Phone Locked, Return for Service".

This is quite frustrating because I'm now in Australia and need to use my phone but can't--and I won't be able to use it when I get back to the States unless we can unlock it.

So even when the customer puts the original SIM back to the phone, it still does not accept and shows "Phone Locked, Return for Service"

If anyone has a solution, please let me know.
