Also which carrier are you trying to use and is the IMEI blacklisted?

imei certi pass , mipi test success , tried att and verzion and cricket, not success

Does it show Network locked when you restart phone with att sim inside?

Check IMEI if not blacklisted

phone clean imei alrady check the imei Tmbile web site and private server , imei clean and ready to activate

blacklisted or hardware problem phone.

phone is still working in the tmobile network, so no blacklist or hardware problem. check please. also tried to repeat unlock for same device and z3x ask for more credits , you said same phone will not ask for credits after unlock done
