Try with latest z3x software version if not done:
Download and flash any firmware include G925FXXU5D with z3x software
then flash modem part from lower version for your action

I have flashing last 6.0.1 firmware and later modem old, but i see always this error :

Try downgrade modem if possible

Unlock?

Then flash all parts from firmware with G925FXXU4xxxx WITHOUT Bootloader and CP part [ aka modem ] . After this load on AP line combination FW COMBINATION_OXA_FA50_G925FXXU5APK6_OXA5APK6_CL6658 768_QB11572664_REV02_user_mid_noship.tar and select to flash ONLY modem ...

Wait process to finish, let you see phone at adb and make unlock [simple unlock, without root ... ] - all be fine.

You can update to Android 7 [ G925FXXU5xxxx ] firmwares after this... Phone will remain unlocked.

P.S. In case after combination modem is used for unlock, both jobs will work - read codes and unlock [old way, simple old way, without rooting] , BUT reading codes will give you wrong result - codes will not work, so use direct unlocking this time.
