appears like be a problem with your box. If is out of warranty you can use a smartcard reader and other box as interface, is bad but you still can use z3x software

unistall restart pc disable ur antivirus then install again if not success plz change usb cable with new one

looks like low voltage, when this issue show in my pc, i change cable or use back usb ports

uninstall software and drivers ,restart pc , download latest shell ,install drivers ,run card wizard and install latest version 15.1
