Help with SAM on 3Gs with new bootrom

Hi all,
first post and first Iphone :-)
I have an iphone 3Gs which I have succesfully jailbroken using redsnow and IOS4.1 and had to update to baseband 6.15 as I have the newer ROM. (Didn't want to go to 4.2.1 as its still a tethered JB).
I then installed SAM and succesfully de-activated the phone.
However, I am unable to get itunes to activate it. I keep getting a "wrong sim card" message from itunes.
I am afraid that I do not know who the original carrier is for this phone.
I do have the model number however MC637B/A as well as the serial number and IMEI number.
It is a UK phone and I have been through most of the options when choosing "Method" in SAM as autodetect did not work either.
So anyone got any ideas?
Any way of finding out which carrier it was originally with?
Or does SAM not work with this version of bootrom?
Thanks.
Mac.

You need to select method "By Country and Carrier" then select United Kingdom and start with carriers who sales iphones in UK, for example o2.... the go to "SIM ID" and click twice and try on every id (5x numbers code) solutions. When you click on every id you'll see phone will start searching network, and then connect usb cable and try to synchronize. If not success then try with other carrier and reconnect usb cable

Still no luck.
Can I clarify a couple of things?
My SIM is with T-mobile.
When trying all this do I need to have the SIM card in or out? Or does it make no difference?
Secondly The only time the phone starts "searching for a network" is when I plug the SIM card in.
It does not search at any other time in the process.
Any further ideas?

@darksman I have also deleted the "activation records" entry as described and still no difference.
I don't have an issue with the unlock its just that I can't activate with itunes without "hacktivation" in redsnow.
Thanks.
Mac.

One other thing of note.
When accessing all the UK carriers they all begin with SIMID numbers 2xxx NOT 5x.
Is this significant?
I am also running itunes 10.1.1.4
Thanks
