what do i have to do to install apps directly using itunes?

im pretty sure i have to have the phone jailbreaked and unlocked but what else is necessary?

with the 4.2.1 is possible?

i searched for it and i found that i have to use appsync for 4.2 OS i will try it and post results

installed appsync for 4.2 os but i dont see this software in the desktop of the iphone,how to use it and syncronize with itunes to try and install the apps from the pc?
