iPhone 4 is stuck in guided access mode - GSM-Forum

My iPhone 4 is stuck in guided access mode on urdu Dictionary app.
it just shows the "Guided Access Mode Enabled.
"Triple Click Home Button to Exit."
but not fix it.
I can not do anything. Help me please.
