your box eeprom is demage you need repair eeprom and use thsi tool your prb solve 100%

that was happen me before.When you click the DCTxBB5 it will prompt two serial number, just because before it happens you plug another box aside from the serial number you installed before.

try this solution and do not install 2 box in one cpu as long as it has hwk orig and follow this procedure:
How to properly upgrade UFS box firmware .

1. Download and run UFS3Tool.exe, Connect>> select v2.6>>
check Write Firmware, Write Eeprom, Write 93c46 and
click Fix and Update buttons.
2. Reconnect box again>> select v2.5>> check only Write
Firmware and click Fix and Update buttons.You have a
downgraded UFS box with v2.5 firmware.
3. To upgrade it to v2.6 or v2.8, run UFS_DCTxBB5.exe -
DCTxBB5 TOOLS v.2.0.1. HWK by SarasSoft or higher,
you can use either original or patched.
4. Connect, and on warning message, choose Yes.
5. Now, your UFS box firmware is properly upgraded .
