The software upgrade process may have installed and registered a new version of bbdevmgr.exe and bbdevmgrps.dll in this directory - C:\Program Files\Common Files\Research In Motion\USB Drivers.

If the version in this directory is 4.1.0.11 (as it was for me) you need to downgrade to an earlier version of both files - such as 4.1.0.5.
