Blackberry call fail $ general error - GSM-Forum

(call fail)i gets this error when when i make call,but when i tries to get account balance it shows (supplementary service error:general error.)but i ve removed it policy and reinstalled os still the sam problem.i also ve gone true the frequently asked questions section but nothing regarding this errors are there.pls what do i do?
