If Your phone is P500h, please select P500h model and write "LGP500h-FOR-UNLOCK", If Your phone is P500 - use "LGP500-FOR-UNLOCK" firmware.

Please, put the phone into Emergency Mode manually and connect it via Micro USB cable to PC, then write here, what the device exactly appears in PC Device Manager.
