Check your JTAG Connection.

Make sure battery is not connected.

Make sure USB Cable is connected properly.

Recheck pinout and resolder properly.

Also check if phone is GROUNDED or maybe water damaged.. in that case, jtag will never work even if pinout soldering is perfect.
