Ok, here is list of reasons why this could happen:

1. Dead eMMC chip
2. Bad connection of NRST line
3. Bad connection of GND line
4. Gremlins

Can You please elaborate a bit phone history and whats the condition of it now ?

on usb is show as qd-loader 9008

My client says that :

phone have unloced boot and custom recovery
this problem was after insert mmc card and reboot phone.

other history not known

nrst and gnd chack many times

Well, it appears that this is hardware fault with eMMC chip.
