it's driver problem i have faced this and when installed the corect driver phone falsh succeed :
Core : v2.03 , NXP FlashEngine : v4.6

Flash Mode : Full Update (System, User, Boot)
Check and select files according selected FlashMode

What system you use Xp or Win7??
delete drivers -> in command dos line type :
set devmgr_show_nonpresent_devices=1 and type : devmgmt.msc
tick on show hidden devices and remove recently installed driver , reboot system hold down button and plug usb when window appear for new decteted driver give path for driver that i have attached here

i did all that said above.....but same problem.....
i think it's qualcomm cdma phone and it's hw version is hw v1....
in support area only v2 available...
