Please try to re-install Qualcomm driver manually from Software installation folder and then repeat the operation again. Connect the device directly to PC motherboard USB port, don't use any USB hubs, this is important. Try to repeat the operation a few times. Also try to connect and repair this device using the latest version of Medusa Pro Software (https://medusabox.com/eng/download/software). Post here the result.
