Most likely, device's eMMC has HW issue. Have You tried to re-connect the device again and do repair? Try to do repair and post here the result.

Unfortunately, Write/Update eMMC firmware for Q313MB (KMQ310013M-B419) eMMC if not suppotyrd at the moment.
