1. Check soldering of all Jtag wires, resolder them, make them as short as possible.
2. Also, as You can see, there is a problem with powering the device:

VREF = 0.0V

VREF = 1.2V
Quote:
VREF = 0.39V
Chech power supply, that You uses - fully charge the battery by using external charger. Also use USB cable, connected directly to PC motherboard USB port (don't use any USB hubs, this is important).

Make sure you have a good connection to VREF from the RJ45 Cable. Do not jumper.
You must have a NICE SOLID connection.

Solder really well to PCB and inspect.
