Hi,
Try to select different eMMC Voltage settings in software (1.8, 2.8, 3.3), make sure that GND line is short and has low resistance.
Also in some cases pull-up resistor is helpfull:

wich adaptor u use its low resistor problems (268 mV) too low

It is a big deal believe me specially cmd and gnd lines must be short as possible and make sure your pinout is correct, also could be a dead emmc or need pull up resistors, what is the condition of the phone
