I am trying to unlock this phone and its shows the following:

build number is N960USQS3CSJ1

It seems The octoplus software is not connecting to my phone via adb when I press on unlock

check attachment showing a screenshot in device manager, I dialed *#0808# and selected dm+modem+adb

Please, close all other Softwares which can use or affect on ADB devices, after repeat operation again, plesae make sure that USB debugging is ON

I closed everything. I even tried on another computer which does not have anything else installed

Well ADB works fine for your device, as You can see Software read out information from your device via ADB.

Please, repeat Unlock ATT operation according to Software manual:

Take out (remove from the phone) SIM and SD cards.
Power the phone On.
Connect powered-on phone to PC via USB cable.
Keep phone"s LCD On and unlocked during the procedure (touch LCD is needed to keep it ON during the Unlock process), this is important!
Click "ATT Factory Unlock" button.
Follow software instructions (prompts).
