Software can't communicate with the phone via diagnostik port, if phone's modem part is damaged.

Try to read out phone's info via ADB (turn on USB Debugging Mode in the phone and press "Read info" button in the Software) and post here full Software log with this info. We will look and will advise You, what to do the next.

As You can see from your Software log, phone's modem part is damaged:
Quote:
SW version: Unknown
This can be Software (modem firmware is damaged, or You have written unsuitable firmware) or HW problem.

You should re-flash the device with different firmwares and check, whether modem is correct (not Unknown). And only then (when phone's modem part will be correct) perform "Repair IMEI" operation. It is not possible to repair IMEI, if phone's modem part is damaged.

Most likely, the problem is with phone's eMMC flash chip.
