As You can see from your Software log, phone's modem part is damaged:

This can be Software (modem firmware is damaged or You have written unsuitable firmware) or HW problem.

At first, try to re-flash the device with different firmwares and check, whether modem is correct (not Unknown). And only then (when phone's modem part will be correct), perform "Repair EFS" and "Repair IMEI" operatios. It is not possible to repair IMEI, if phone's modem part is damaged.

Try to write 4.1.2 firmwares from the Support Area one by one.

To put GT-I9100 phone into download mode you have to perform the
following steps:
1. Reconnect phone battery.
2. Press and hold "Volume - " + "Home" + "Power" buttons.
3. When phone turns on press "Volume+" button.

Try to write different firmwares, not only one. If after of writing all firmwares, which You have, phone's modem part will be still Unknown - most likely, this is HW problem (the problem is with phone's flash chip).
