As I see from this log, modem part is damaged in your phone:
Quote:
SW version: Unknown
This can be Software (modem firmware is damaged or You have written unsuitable firmware) or HW problem.

At first, try to re-flash the phone with different firmwares and check whether modem is correct (not Unknown) and only then perform "Repair EFS" and "Repair IMEI" operations.

Also, please, be attentive and always select correct device model:

Try to write different firmwares from the Support Area. Start with "I9500UBUAMDK_ZTOAMDK_Brazil_Android_4.2.2".

As I see from this log, modem part is still damaged in your phone:
Quote:
SW version: Unknown

Try to write different firmwares from the Support Area one by one and check, whether modem is correct. If after of writing all firmwares, phone's modem part is still damaged, most likely, this is HW problem.
