Hello.

1.
As You can see from this Software log, modem part is damaged in your device:
Quote:
SW version: Unknown
This can be Software (modem firmware is damaged or You have written unsuitable firmware) or HW problem.

At first, try to re-flash the device and check whether modem is correct (not Unknown) and only then (when phone's modem part will be correct) try to perform "Repair IMEI" operation. It is not possible to repair IMEI, if phone's modem part is damaged.

2.

When phone's modem part will be correct, You need to connect the device correctly. Phone's info should be read fully and correctly. In your case, the phone is not connected correctly:

Firmware compiled date:
Firmware compiled time:
Firmware released date: -ù~

If You connect the device correctly, phone's info should be read like here:
Quote:
Firmware compiled date: Mar 28 2013
Firmware compiled time: 09:00:00
Firmware released date: Apr 12 2013
Firmware released time: 00:04:45

This means You have not connected the phone correctly. You need to connect the phone correctly, then read phone's info. And only then, when phone's info will be read correctly and fully, perform the Repair IMEI operation.

To connect GT-I9505, you have to perform the following steps:

1. Disconnect USB cable from phone.
2. Go to Menu->Settings->More->About Device.
3.Tap on "Build number" 7 times to enable developer options.
4. Go to Menu->Settings->More->Developer options.
5. Turn on "USB Debugging" option.
6. Go to dial window.
7. Enter "*#0808#" number.
8. Select "DM+Modem+ADB".
9. Press Home button.
10. Go to dial window.
11. Enter "*#9090#" number.
12. Set "Diag config" to "USB" value.
13. Restart phone.
14. Press "Read Info" in software.
