Welcome to Octoplus/Octopus Box Samsung software version 1.6.0
-----------------------------------------------------------------------------------
To connect SGH-I337M phone you have to perform the following steps:
1. Disconnect USB cable from phone. 
2. Go to Menu->Settings->More->About Device.
3.Tap on "Build number" 7 times to enable developer options.
4. Go to Menu->Settings->More->Developer options.
5. Turn on "USB Debugging" option.
6. Go to dial window.
7. Enter "*#0808#" number.
8. Select "DM+Modem+ADB".
9. Press Home button.
10. Go to dial window.
11. Enter "*#9090#" number.
12. Set "Diag config" to "USB" value.
13. Restart phone.
14. Press "Read Info" in software.

Hello.

Please do the next:
1. Write firmware "I9505XXUFNA5_I9505OXAFNA5_I9505XXUFNA5_OXA_4. 4.2" from Support Area.
2. Write modem file "I9505XXUFNA1-MODEM" and GT-I9505_for_Unlock_and_IMEI_repair.oct".
3. Root the device.
4. Reboot the phone.
5. Connect powered on phone via Micro USB cable directly to PC (don't use USB hubs etc.).
6. Enter correct IMEI.
7. Click "IMEI fix" button.
8. Follow software instructions (prompts).

Please perform speps 1-8 mentioned above. Also, to connect GT-I9506, You have to perform the following steps:

1. Disconnect USB cable from phone.

This means your device is not rooted correctly or USB Debugging Mode is not turned on. Please disconnect all other device from the PC, re-boot the PC, re-start the phone, turn on USB Debugging Mode and repeat the operation again.

As You can see from your Software log, phone's modem part is damaged:

This can be Software (modem firmware is damaged or You have written unsuitable firmware) or HW problem.

You should re-flash the device with different firmwares and check, whether modem is correct (not Unknown). And only then (when phone's modem part will be correct) perform the "Repair IMEI" operation. It is not possible to repair IMEI, if phone's modem part is damaged.

If after of writing all firmwares which You have, phone's baseband is still Unknown - most likely, this is HW problem (problem with eMMC flash chip).
