What is the history of your device? After of which operations exactly your device got this issue? Are You sure this is not HW problem?

Try to make Repair EFS, Repair IMEI, Unlock and re-flash the device.

Welcome to Octoplus/Octopus Box Samsung software version 1.5.7

To connect GT-N7000 phone you have to perform the following steps:
1. Disconnect USB cable from phone.
2. Go to Menu->Settings->Applications->Development.
3. Turn on "USB Debugging" option.
4. When message is appear press "Yes".

7. Set "USB" radio button to "PDA" value.

As You can see from your Software log, phone's modem part is damaged:

This can be Software (modem firmware is damaged or You have written unsuitable firmware) or HW problem.

At first, try to re-flash the device with different firmwares and check, whether modem is correct (not Unknown). And only then (when phone's modem part will be correct), perform "Repair EFS" and "Repair IMEI" operatios. It is not possible to repair IMEI, if phone's modem part is damaged.
