As You can see from your Software log, phone's modem part is damaged:

SW version: Unknown

This can be Software (modem firmware is damaged, or You have written unsuitable firmware) or HW problem.

You should re-flash the device with different firmwares and check, whether modem is correct (not Unknown). And only then (when phone's modem part will be correct) perform "Repair EFS" and then "Repair IMEI" operations. It is not possible to repair IMEI, if phone's modem part is damaged.
