As I see from this log, modem part is damaged in your phone:
Quote:
SW version: Unknown
This can be Software (modem firmware is damaged or You have written unsuitable firmware) or HW problem.

At first, try to re-flash the phone with different firmwares and check whether modem is correct (not Unknown) and only then perform "Repair EFS" and "Repair IMEI" operations.

Try to make "Repair EFS" and then "Repair IMEI". If after of "Repair IMEI" (or "Repair Network" also) operation, phone's baseband becames NULL (SW version: Unknown) with different firmwares, most likely, your device has new HW revision. Currently no solution for such devices.
