Please put the phone into Download Mode and connect USB cable.
To put SGH-I747M into Download Mode, You have to perform the following steps:
1. Reconnect the battery.
2. Press and hold "Volume-" + "Home" + "Power ON" keys.
3. When phone turns on, press "Volume+" key.

To connect SGH-I747M phone you have to perform the following steps:
1. Disconnect USB cable from phone.
2. Go to Menu->Settings->Applications->Development.
3. Turn on "USB Debugging" option.
4. When message is appear press "Yes".
5. Go to dial window.
6. Enter "*#7284#" number.
7. Set "UART" radio button to "PDA" value.
8. Go to "Qualcomm USB Settings".
9. Select "DM+Modem+ADB".
10. Press Home button.
11. Go to dial window.
12. Enter "*#9090#" number.
13. Set "Diag config" to "UART" value.
14. Restart phone.
15. Press "Read Info" in software.

As You can see from your Software log, phone's modem part is damaged:

This can be Software (modem firmware is damaged, or You have written unsuitable firmware) or HW problem.

You should re-flash the device with different firmwares and check, whether modem is correct (not Unknown). And only then (when phone's modem part will be correct), perform Repair IMEI operation. It is not possible to reapair IMEI, if phone's modem part is damaged.
