Try to do the next:
1. Make Factory Reset and reflash the phone.
2. Repeat "Unlock" operation.
3. Try to enter 00000000 and 12345678 codes.

Post here result and full Software log of performed operations.

Please do exactly the next:
1. Reflash the phone with "j700hxxu1aogb_j700holb1aog1_xxv_android_5.1.1 " firmware from the Support Area.
2. Root the phone.
3. Repeat "Unlock" operation.
4. Try to enter 00000000 and 123465789 codes, if will be needed.

Post here results and full Software log of these operations.

Please upload *.log and *.logx files of "Unlock" operation, performed after You wrote "j700hxxu1aogb_j700holb1aog1_xxv_android_5.1.1 " firmware and post here download link. We will check where can be the problem.
*.log and *.logx files can be found in "C:\Program Files (x86)\Octoplus\Octoplus_Samsung\LOG" folder.
