If every time on same adress mean FLASH IC damaged.
Try rehot it by hot air and after try flash.

Try to flash another firmware, to exclude firmware files corruption during the download.

Its connectoer prob,
use another usb cable ( I used SE orginal isb cable ) and keep Hold and Push the cable into Phone during the Flash,do not relase the cable before finshing FLASHING,
it will fix ur prob.
