Verizon USA iPhone 4S Unlock Clean 99xxx READ INSIDE!

I'm looking for unlock service for my Verizon USA locked iPhone 4S 99xxx IMEI.
The phone is clean and off contract. It's been replaced once under warranty.
I need to unlock but tried several clean services with no luck.
Some resulted UNLOCKED but NOT accept sim card, some resulted NOT FOUND!

This one is cdma phone. You cannot unlock this mobile

It has sim card slot and if you unlock you can use overseas with sim card. 4s cdma have sim slot. 4g cdma don't have. It's 4s bro.

iPhone 4S is a world phone. Dual antenna system. Both GSM and CDMA. (qualcomm's dual capable baseband chip)
Better watch the Apple's 2011 keynote
They mentioned it.
Only iPhone 4 have 2 different versions. Only GSM and Only CDMA. But the CDMA verzion don't have SIM slot.

And as you can see in the GSX report posted in the item description section there are MM shown. That means Multi Mode! So I need to change next tether policy to Multi Mode Unlock

So I figured out whats the problem with this case of mine.
I have one of replaced verizon locked iphone 4s in hand.
According to verizon usa unlock policy they don't sell locked phones. So customers can use their phones with any sim outside us if they travel.
So in my case the original phone which was sold by verizon is unlocked but apple replaced the device under warranty and set the wrong policy. Now phone only works in CDMA verizon network.
Because the replaced imei number is not in any of the database except apple nobody can unlock it except apple level2 or level3 engineer.
This kind of employee can access to policies database at apple because this database is not synced with carrier database. So carrier can't help if your policy is service policy. Next tether: any service policy.
