Check soldering of all JTAG wires, make them as short as possible and try to connect again. Try to stabilize VREF - use fully charged battery, also try to use USB cable for powering the device; try to connect USB cable to different PC USB ports (don't use USB hubs, this is important). Post here full Software log, not reduced.

Try to connect the device using Medusa Pro Software and post here full Software log.
