To connect the phone in Normal Mode, do the next: Select "Charge Phone" in "USB PC Connection" menu. Type ##3424#, go to "Diag" menu and select "Enable".
Please, ake sure that You select Charge only type of connection.

Please, post here full Software log of Unlock operation for this device, we will check it.
If You will perform Factory Reset, sprint phones will be locked again, to unlock your device, You need to repeat Unlock operation.

Please, try to reflash your device, but make sure that You aren't downgrade phone's firmware, downgrade can damage your device.
