If problem will remain, then, most likely, this is HW problem.
Please fully charge phone's battery and try to perform "Write Firmware" operation again, with different firmwares.

Also, try to re-flash the phone with Nexus 5 Root Software, which is available in "/LG/FOR UNLOCK AND REPAIR/" folder of the Support Area.

If problem will remain the same, then, most likely, your phone has HW problems (with phone's flash chip).

flash with nexus root toolkit use firmware 4.4 not use 5.1...after flash baseband is unknown try flash with octopus and u phone is normal..
