Try to reflash the phone with original firmware and flasher. If problem with writing of a firmware will persist - most likely your phone has HW problem.

You can find necessary files and instructions by the following link: https://developers.google.com/androi...s/images?hl=en
Also, You can try to reflash the phone via "nexus 5 root software", which is available in "lg/for unlock and repair" folder of the Support Area.
